I would take him to the vet, he may have worms, or an infection, kidney stones, it could be a million treatable things. If you and your vet decides it is not treatable, or not reasonably treatable, then you will have that tough decision to make

I am sorry your kitty is not doing so well. 14 is not *that* old for a cat (many live til 17-18) and there are lots of medical conditions that could lead to those behaviors. Avoiding the litterbox is a classic kitty cry for help. When my 12 pound cat suddenly dropped weight to 6.5 pounds, I learned he had kidney failure. I thought he was a goner but with simple medical care, he's lived another good 18 months and is still going (although not particularly strong these days). . .

Although I am not a cat person, Cognitive Dysfunction Syndrome is also exhibited in cats. Our 14 yr. old OES Maggie was diagnosed last fall and her improvement after being treated with Anipryl is amazing. Prior to treatment, she continually wandered in circles and got stuck behind furniture and was generally in a daze. Sometimes forgot to go outside to potty.. May be worth researching this condition as a possibility.
