MDR-1 mutation - sensitivity to drugs in OES

Hi all - some of you have read my posts regarding our dog Bo's eye problems, but I found something in doing my research that I thought others might be interested in as well. And forgive me if this is old news and I'm just late to the party! But when I was discussing Bo's retina issues with the veterinary optholmologist today I mentioned the fact that he was on Comfortis for fleas when we first rescued him this summer - we had a hard time knocking out the fleas with just frontline. He only took two doses. She told me that she has seen a few cases of blindness related to Comfortis - so I googled it, and found an FDA warning about using Comfortis with Ivermectin - which is basically Heartguard. That googling let me to a discussion of a gene mutation in dogs called MDR-1 - Washington State University has done alot of research on this and has developed a test so see if your dog has this gene mutation. It seems that if your dog has it they are mostly likely to have adverse reactions to certain drugs including ivermectin and imodium. OES are one of the breeds at an increased risk of having this mutation - about 5% of OES will have it, according to their website. Anyway - I just wanted to mention it -- it seems as though if you have an OES and give Comfortis you should probably not give it at the same time as ivermectin. Or at least talk to your vet -- and I know I may not give imodium after reading this - or maybe even get Bo tested. It costs $70 to test your dog. I just took my new puppy Dahlia to our vets tonight and she gave me a listing of this and discussed this with me.

You can get a cheek swab or some sort of test done and they will send it in so you can find out if your OES has this mutation or not.

I had no idea that Comfortis had Ivermectin in it!

how they can sell in over the counter since
Ivermectin at a vet you have to have a prescription...

Oh, sorry, I think I confused you -- Comfortis does not have ivermectin in it, but it can interact wtih Ivermectin -- and I think this problem is true for all dogs, not just those with the gene mutation. But I know in my case, since the comfortis was given monthly, and so was the heartguard (ivermectin) I gave them at the same time -- and if you have a dog with the gene mutation, that could be asking for trouble. The FDA warning (you can google it and find it) says that the heartworm prevention dose of ivermectin is safe with comfortis - but it would make me a little uneasy about giving them together -- and if you are giving ivermectin at higher doses (i.e. for treating mange), then you should definitely not use comfortis. hope that clears things up! sorry about that!
Yes, MDR1 is a concern w/ OES, as part of the herding group of dogs.
Our dogs have all been tested, and it's quite a simple test.
We did a group (grown littermates) a few years ago. I know at least 6 of the 8 littermates were done. I swabbed the 3 in MN, and they got sent in w/ the 3 in WI.
Ours are normal/normal, and I know 1 in the group was mutant/normal (mutant/mutant is what you don't want).
It's a good test to have done.
There have been several discussions on here about it, but this is certainly a valid one worth bringing up again :D

There is a very good & imformative article in the March 2011 OET on drug sensitivity for those of you who get the OET. Having a boy who has a sensitivity to Flagyl, this was very intersting to me.
BTW, I did have Dahlia tested for this and it was fine. She doesn't have the mutant gene. I am glad I did it since she will be spayed at some point and I have a little piece of mind that she won't have a reaction to the anesthesia.

Not intended to scare you in any way - a reaction is very unlikely - but one of my girls reacted very badly to ketamine administered when I had her OFA x-rays taken. My vet suggested I get her tested for the MDR1 mutation "just in case" ( I don't think ketamine is one of the drugs listed among those MDR1 affected dogs have sensitivities too, but...?) She came back normal/normal. So just because they don't carry the mutation, and I think it's great that you checked, there can be other drug sensitivities for reasons as yet unknown.

Unlikely, mind you. Just mentioning it as an outside possibility.

My vet is switching from carrying regular Comfortis to Trifexis, which is a combination of Comfortis (flea control) and Interceptor (heartworm and intestinal parasite protection.)


I don't plan on giving my dogs the combo- (we don't really have a heartworm problem here)
but I would be concerned there would still be a reaction.

Is Interceptor that same as ivermectin?
