Not totally housebroken yet!

Butch is almost 9 mos old, he is doing pretty well on the house breaking but still has his lapses. He doesn't have any clear cut signal so unless your watching him pace you miss it. Have tried the bell on the back door etc, he doesn't get it. The one main problem is a small bedroom, I would like to leave the door open and he does well for several days and then he thinks he should go in there and either pee or poop on the bed.
Any suggestions would be appreciated!!!!!!!!!!!!!!!!!!!!!!!
